At CHS we support community innovation for health improvement.  We use a proven framework we call the ‘community innovation model‘ to help people assess needs, design strategies, develop capacity, execute for results, and demonstrate value.  This framework can be applied to support a single organizations or a group of organizations working together in a collaborative context.

Community strategy begins with a shared understanding of community needs and assets.  We can help you:

  • Define the community focus for the initiative
  • Develop quantitative indicators of community health and well-being
  • Generate qualitative insights about community needs, assets, gaps, and opportunities
  • Summarize results in efficient reports and visualizations
  • Use results to inform community strategy development

The results of the community assessment can be used to inform development community strategies to address specific issues.  CHS can help you:

  • Engage key stakeholders as strategy partners
  • Define a shared vision, a set of values, and set of priorities for creating community impact
  • Create strategic goals, an action plan, and a funding plan
  • Specify intended results and key performance indicators
  • Design a collaborative structure for guiding and managing the effort.

Collaboration is essential for solving complex community problems. At CHS we work with our client partners to create community collaboratives that help people build capacity for community health improvement.

  • A community collaborative is a planned program for engaging teams of people from multiple organizations in collaborative learning and action to accomplish defined goals.
  • A community collaborative can be convened by a foundation, an association, or another organization seeking to foster collaboration for learning and action.
  • The basic approach is to choose a focus, engage participants, define goals, design and develop supports, and collaborate on learning and action to accomplish the goals.
  • The number of organizations participating could range from three to dozens depending on the scope and nature of the project.
  • The time period could run from a few weeks to many months, depending on the scope and nature of the project.
  • The supports and activities are tailored for the project, and typically include a mix of online resources, virtual meetings, in-person strategy sessions, coaching, and technical assistance.

Community initiatives require focused execution aimed at optimizing results.  CHS can help you:

  • Assure that community partners are equipped and ready to execute the initiative
  • Assure that essential inputs and resources are available
  • Assure that key strategies are being executed
  • Assess whether key outputs, outcomes and impacts are being achieved
  • Make strategic adjustments to the community initiative based on feedback and evaluation.

In today’s funding environment it is essential to define and demonstrate value to funders, partners, and community members.  CHS uses a specialized model of evaluation called ‘community value analysis‘ to help define and demonstrate the value of community initiatives in their own local context.  We can help you:

  1. Define your objectives for demonstrating value to key stakeholders
  2. Identify specific audiences who should know about the value of your organization or initiative
  3. Create the value messages (what you want them to know) for each audience segment
  4. Develop value indicators (quantitative and qualitative indicators) to support the value messages
  5. Design a communication plan for each audience segment
  6. Execute the communication plan for positive results

Action learning is a powerful strategy that brings people together to build skills, solve challenges, and create a sustainable capacity for excellence. Action learning combines strategy consulting with learning support to assure that the knowledge generated from the project stays with the organization after the consulting team leaves.  The results can be seen in the solutions produced plus the sustainable capacity for excellence resulting from the action learning process.

At CHS we support collaborative action learning projects for community initiatives.  The value of these projects lies in bringing multiple organizations together to share  learning and take action to innovate and solve problems.  Together we can develop a collaborative action learning program to help your community initiative accomplish its goals for capacity building and positive impact.
