Stephen Horan
Stephen Horan, PhD is the founding president of Community Health Solutions, bringing more than 25 years of professional experience to each client and project. Under Steve’s leadership Community Health Solutions has helped hundreds of organizations achieve better results through focused assessment, design, capacity building, and execution. Much of this work has been accomplished through Action Learning Networks, a pioneering solution for helping organizations learn while solving immediate challenges in health policy and practice. Individually, Steve has been an advisor to consumer groups, nonprofit organizations, corporations, foundations, and public sector leaders. Terry Cyr
Terry Cyr, MS has been with Community Health Solutions since it began in 2001. She currently serves as Director of Operations for Community Health Solutions with a focus on ensuring all of our products and tools are practical and useful to our clients. With more than 20 years of professional experience in various aspects of health care planning, research, and administration, Ms. Cyr has particular experience in supporting leadership development and health quality/care delivery projects for nurses, physicians and interprofessional healthcare teams. She completed IHI’s Breakthrough Series College and applies learned strategies to help our clients achieve collaborative health quality innovation improvement aims. She achieved certification as a NCQA Patient-Centered Medical Home Certified Content Expert (PCMH CCE) in the fall of 2015. Terry holds a Master of Science Degree from James Madison University. Caitlin Feller
Caitlin Feller, MPP, PCMH CCE is a Principal at Community Health Solutions. Caitlin currently leads projects in health care quality improvement, patient-centered medical home (PCMH) development, chronic care improvement, and integrated oral and behavioral health care. Through these projects, Caitlin provides practical, hands-on support for dozens of clinician practice teams working on improving and sustaining quality. She also provides training, technical assistance, and analytic support to help health care organizations demonstrate their value to key stakeholders. Many of these projects are conducted through collaborative learning networks, whereby groups of individuals and organizations work across sectors to learn and implement effective policies and practices for health improvement. She achieved certification as a NCQA Patient-Centered Medical Home Certified Content Expert (PCMH CCE) in the summer of 2013. Sherrina Gibson
Sherrina Gibson is a Principal at Community Health Solutions. Sherrina currently leads projects in health analytics, and provides support on projects involving quality improvement and population health. Her areas of expertise include evaluation, data development, and healthcare issues of the Medicaid and uninsured populations. Sherrina brings nine years of experience to the team, and holds an B.A. in Sociology. Kim Waters
Kim Waters is a Project Support Analyst at Community Health Solutions. Kim received her B.A. in Healthcare and Society from the University of Richmond in May 2015. She is currently assisting on projects involving population health, quality improvement and patient centered medical home (PCMH) development. She works in the area of data analysis and visualization using a variety of software including Tableau. Her community interests include Emergency Medical Services – Kim is a certified EMT.
